Sixth Form Application Portal

Relevant Sixth Form Information

  • Applications to the Ascot High School Sixth Form Programme can be made through our online application portal or using our physical application form available at the school's main office for JA $1500.
  • ONLINE APPLICATIONS ARE FREE OF COST!
  • Current grade 12 students who wish to continue the programme do not need to re-apply but they must qualify to re-register in the programme. 
  • All applicants must meet the entry requirements to sixth form before making an application. 
  • Once you start your application, you can save it and return to it at a later date. However all required information must be provided before you can submit the application. In fact you must successfully complete each section before you can proceed to the next. If you are awaiting results to complete your application, you will not be able to submit the application without those results.
  • The following are the required supporting documents for ALL APPLICANTS.
    • A Passport Size Picture (2 inches by 2 inches)
    • Completed Conduct Assessment Form. Download the form here
    • Two recommendation letters (Academic Reference and another from the community: JP, Lawyer, Pastor, Police)
    • A Copy of your Birth Certificate
    • Proof of Examination Results
  • While you are encouraged to scan and upload the supporting documents, you will be allowed to complete the application online without uploading them; however, you will be required to submit them later at the school's main office.

Entry Requirements to Sixth Form

Academic Requirements for Pathway ONE

  • 1. All applicants must have a pass in English A and Mathematics at the CSEC or GCE level. Special considerations can be made on a case-by-case basis for exemption.
  • 2. All applicants must have at least five CSEC and/or GCE Subjects. Applicants must have at least a Grade II in pre-requisite subject in the case of a CSEC Subject or a Grade B in the GCE Subject.
  • 3. All applicants must be qualified to pursue three CAPE subjects and Communication Studies. All students will only be allowed to do a total of FOUR SUBJECTS. Applicants must receive permission from the programme coordinator to do a fifth subject.

Academic Requirements for Pathway TWO

  • 1. All applicants must have at at least ONE CSEC/GCE/CVQ/NCTVET/City & Guilds Stage 3 subject. A pass in the HEART Diagnostic Test will be accepted if the applicant does not have any subject.
  • 2. All applicants will be required to do a vocation and three (3) other subjects including Mathematics and English Language. Applicants must receive permission from the programme coordinator to do more subjects.
  • 3. Based on their qualifications, applicants may apply to do CAPE subjects in this Pathway.

Supporting Documents Required for ALL APPLICANTS.

  • 4. All applicants must obtain two recent recommendations (Not older than sixth months, One from a teacher and the other from any of the following persons: Doctor, Lawyer, Police Officer, Justice of the Peace, Pastor)
  • 5. All applicants must complete a conduct assessment form. This form must also have the stamp of your previous school affixed to it.
  • 6. Special admission to this pathway will be considered on a case-by-case basis.

Terms and Conditions

Hereinafter "the school", "us", "we", "our", and "ours" refer to Ascot High School. Additionally, "you", "your", and "yours" refer to the applicant. The "rules" refer to the rules of the school; the "terms" refers to these terms and conditions and the "law" or "the law" refers to the Laws of Jamaica. "Online portal" or "our portal" refers to the Ascot High School Sixth Form Online Application Portal, and "the programme" refers to the school's sixth form programme

    Privacy

  • All information you submit on our portal shall not be shared with a a third party unless the information submitted or the manner in which you submitted the information constitute a breach of the law. In the latter case, the information will only be shared with law enforcement agencies of Jamaica. A third party constitutes someone or an entity that is not connected to the school in anyway whether by law, employment, or having any relationship whether social or economical to school.
  • Cookies used by this site does not seek to gather information about your use of the internet.
  • All the information you submit on this site now belongs to the school
  • Website Servicing

  • We have the right to make the website unavailable at any time to effect any repairs, update, or modifications we deem fit at any time during and/or after the application period. We shall not be liable for the failure of submission of any application to due the unavailability of the site for the purpose aforementioned described
  • Termination

  • You are only authorized to use our portal to make an application to the programme. Any other use that we find to be contrary to the service we provide through our portal, we reserve the right to delete your account, whether or not your application was submitted successfully or not
  • Warranty

  • We will make every effort ensure that our portal remains on-line for the duration the applications are opened to facilitate you making your application to the programme. However, we do not guarantee that the site will remain on-line for the entire time the school is accepting applications. Hence, the school shall not be liable for your failure to make an application to the programme because the portal was not available for any period of time during which the school is accepting applications to the programme. It shall be your responsibility to ensure that you make the application while the site is available.
  • We also do not guarantee the safety of storage of your information submitted. It shall be your responsibility to keep copies of the documents you upload to our portal in case you may need to re-upload them because of site failures or any other reason.
  • We do our best to make our portal safe with the use of SSL Certicate and other safety arrangements. However we cannot guarantee 100% safety.
  • If your application is not submitted before the application period is closed, your application with any supporting documentation will be deleted.
  • Admendments

  • We reserve the right to modify, amend, and annul any section or part of these terms at any time we deem it fit to do so.
